Job summary

Qualifications

• Basic Cardiac Life Support must be obtained within 30 days of employment start date.
• 1 year of Case Manager/Social Worker experience required. Inpatient experience preferred.

Responsibilities

• Work collaboratively with RN case managers, acute care nurses, physicians, insurance companies, and other staff members.
• Identify and address barriers to discharge and recovery.
• Perform thorough assessments and develop appropriate discharge plans.
• Participate in the interdisciplinary care planning process and perform proactive discharge planning.
• Coordinate and facilitate meetings between the care team and patients/families.
