Job summary

Northwest Texas Healthcare System is seeking a Social Worker (BA/BSW) to provide social services and discharge management for patients and families. This role supports quality of continuum care and involves evaluating pre-discharge needs with input from the healthcare team.

Required is a Bachelor’s degree in social work or related field; LBSW is preferred. At least two years of relevant experience in discharge planning or related settings is expected, with a focus on helping patients secure

Qualifications

• Bachelor’s degree in social work, psychology, or related field.
• Licensed Bachelor Social Worker (LBSW) preferred.
• Minimum of two years of relevant work experience (e.g., customer service, case management, or discharge planning).

Responsibilities

• Provides social services and discharge management to patients and families; these functions contribute to the quality of continuum of care.
• Completes pre-discharge evaluations from patient/family interviews and healthcare team collaborations; develops a plan of intervention for discharge needs considering illness, living situation, support systems, and financial status.
