Job summary

HCA Houston Healthcare Southeast in Pasadena, Texas is seeking a Social Worker (BSW) to evaluate psychosocial needs and coordinate community resources for patients. You will work with a multidisciplinary team to implement effective discharge plans across Med/Surg units.

Primary duties include identifying discharge barriers, developing care plans with the RN team, and educating patients and families about post-acute options and coverage. A Bachelor’s degree and LBSW licensure are required.

Qualifications

• LBSW license required for eligibility and practice.

Responsibilities

• Collaborate with care team to identify patient needs and coordinate discharge plans.
• Assess psychosocial needs and provide community resource referrals.
• Coordinate meetings with patients, families and care team.
• Develop and implement discharge plans for high-risk patients.
• Educate patients and caregivers on post-acute care options and coverage.
