Job summary

The role requires a Bachelor’s in Social Work and licensure or eligibility (LICSW, LCSW, LMHC) with at least 2 years in acute hospital settings preferred.

Qualifications

• Bachelor's of Social Work degree required.
• Must be licensed or license-eligible to practice as LICSW, LCSW, or LMHC in accordance with state requirements.

Responsibilities

• Conduct complex psycho-social assessment and intervention.
• Coordinate transition planning and care across multidisciplinary teams.
