Job summary

WellSpan Health is seeking a Registered Nurse with utilization and case management expertise to coordinate patient care and discharge planning in a team-based setting. You will work with social work and the care team to optimize resource use and ensure quality, cost-effective care.

The role requires 2 years of acute care experience, an RN license, and BLS certification, with CM certification preferred. Day shift with occasional weekends may apply.

Qualifications

• Associates Degree Required.
• Bachelor's Degree Preferred.
• 2 years Recent acute care experience Required.
• Field of Experience in Utilization Management, Case Management, or Clinical Nursing Specialty.
• Licensed Registered Nurse Upon Hire Required; Basic Life Support Required; Certification in Case Management preferred.

Responsibilities

• Reviews assigned patients for medical necessity, utilization management, and appropriateness of setting for continued hospitalization.
• Initiates problem-solving to prevent over- or under-utilization.
• Coordinates patient care progression to optimize throughput and resource use.
• Acts as liaison between utilization management, third party payers, and the treatment team.
• Assists with discharge planning and coordination of DME, Home Health, IV antibiotics, etc.
