Job summary

Placements24 in Welkom is seeking a compassionate Disability Support Officer to empower individuals with disabilities to live independently within the community. You will implement tailored support plans and assist with daily living activities, mobility, and access to services.

The role emphasizes dignity, inclusion, patient and empathetic support, and collaboration with families, professionals, and healthcare providers to promote rights and independence.

Qualifications

• Experience in disability support, aged care, or related field.
• Qualification in disability services, community services, or healthcare.
• Understanding of disability rights and person-centered support.
• Excellent communication and problem-solving skills.
• Willingness to travel within Welkom region.

Responsibilities

• Develop and implement individualized support plans in consultation with clients, their families, and professionals.
• Provide direct support to clients with personal care, daily living activities, and mobility.
• Facilitate clients' engagement in community activities, education, and employment opportunities.
• Monitor clients' progress, document observations and changes in needs or condition.
• Liaise with healthcare providers, therapists, and other support services for holistic care.
• Educate and advocate for clients to promote rights, independence, and inclusion.
