Job summary

Great Bay Services in Sanford, ME is seeking a dedicated Case Manager to support adults with intellectual and developmental disabilities. The role is 40 hours weekly, hybrid work with Springvale area, and includes benefits such as health insurance and PTO.

You will collaborate with a care team to coordinate MaineCare, housing, employment, and community-based services. The ideal candidate has a bachelor’s degree in a related field, a valid driver’s license, and at least 3 years of social services

Qualifications

• Bachelor's degree in psychology, social work, or related field.
• LSW or LSX preferred.
• 3 years’ experience in social services.
• Valid driver’s license with clean driving record.

Responsibilities

• Advocate for MaineCare services including medical, housing, employment, and community-based services.
• Meet regularly with members to review progress, health, safety and quality of life.
• Assist with social security, food stamps, and MaineCare applications.
• Coordinate medical, dental, vision, and psychiatric appointments.
• Maintain case records and EverGreen reporting requirements.
• Discuss progress, barriers, and solutions with the individual and treatment team.
