Job summary

Morrison Center in Westbrook, Maine is seeking an Adult Case Manager to support adults with intellectual and developmental disabilities in a community setting. The role involves home and program visits, coordination of care, and collaboration with families and partners.

We look for a compassionate professional with a Bachelor’s degree and at least 1 year of relevant experience; knowledge of DHHS regulations and strong communication is essential.

Qualifications

• Bachelor’s degree in any field.
• Minimum 1 year of paid experience with individuals with intellectual and/or developmental disabilities.
• Strong organizational, writing, and time management skills.
• Ability to thrive in a fast-paced environment with multitasking and problem-solving.
• Previous experience in adult case management is preferred but not required.
• Microsoft Office Suite.

Responsibilities

• Complete home visits, program visits
• Case load between 22-25 clients. Some transportation is required.
• Working one-on-one with adults in a community setting.
• Facilitate team meetings
• Complete Person-centered assessments, comprehensive assessments, 90 day reviews.
• Develop and implement individualized support plans using a person-centered approach
• Analyze services and support strategies to ensure they meet the client’s unique needs and goals
• Guide and advocate for clients as they navigate community resources, supports, and opportunities
• Maintain accurate and timely documentation in compliance with DHHS regulations
• Serve as a trusted liaison between clients, families, and community partners
• Represent Morrison Center with professionalism and compassion in all settings
