Job summary

Qualifications

• Bachelor's degree or equivalent experience in Human Services, Social Work, Psychology, or related field.
• Excellent communication, organization, and relationship-building skills.
• Ability to work independently and collaboratively with a multidisciplinary team.

Responsibilities

• Manage a caseload, build meaningful relationships, and develop IPPs.
• Advocate for participant rights, choices, and personal goals.
• Coordinate services with families, guardians, providers, and community resources.
• Monitor progress, adjust supports, and provide guidance and problem-solving assistance.
• Maintain accurate documentation, make referrals, and collaborate with teams to achieve positive outcomes.
• Participate in agency initiatives to foster a respectful, compassionate environment.
