Qualifications

• Bachelor’s degree in Human Services or related field from an accredited college or university.
• Associate’s degree with four years of work experience working directly with persons with disabilities.
• Must be at least 21 years old.
• Valid Pennsylvania Driver's License with acceptable driving record.

Responsibilities

• Coordinate care across the program and maintain documentation.
• Develop and implement Individual Service Plans with client, family and team.
• Facilitate ISPs and IPT meetings and ensure timely progress notes and updates.
• Serve as primary contact for families, agencies and professionals involved in care.
• Coordinate admissions and discharge with program leadership and housing staff.
• Arrange and transport clients to appointments and advocate for client safety.
