Job summary

StepStone Family & Youth Services is hiring for an operations-focused role dedicated to coordinating referrals and managing case processes to meet client needs in our service areas. You will liaise with clinical leadership to determine feasibility and ensure timely pre-screening and admission activities.

The role requires strong communication, a valid driver's license, and at least two years of case management experience within human services.

Qualifications

• Bachelor's degree in a human services or nursing field.
• Valid driver's license.
• Two years of case management or related experience.
• Strong verbal and written communication skills.

Responsibilities

• Receives and responds to incoming calls from referral sources and potential clients to identify needs and consults with Director of Clinical Management.
• Completes referrals by setting up pre-screening assessments, preparing reports, and coordinating pre-admission paperwork and authorization for payment.
• Oversees and supervises field staff assisting with pre-screening processes.
• Accesses account information and past payer agreements as appropriate.
• Consults with third party representatives on client benefit coverage and reimbursement procedures; negotiates levels with payers.
• Contacts referral sources to advise case acceptance and provide information on the clinical team responsible for the client.
• Maintains knowledge of all services and community resources; identifies alternative sources when needed.
• Maintains relationships with referral sources and payer case managers; tracks satisfaction and conversion metrics.
• Implements operational processes to ensure compliance with policies and regulatory mandates; participates in HIPAA and ethics programs.
• Participates in growth planning and special projects as assigned.
