Job summary

Homewood Health is seeking a Case Manager to join a collaborative, inter-disciplinary team focused on delivering seamless services and programs to clients. The role involves adjudicating disability absences, facilitating return-to-work, and coordinating referrals with insurers and health professionals.

Ideal candidates have 2–3 years in disability management or health fields, a relevant degree, and strong communication and analytical skills.

Qualifications

• 2–3 years' experience in disability management or health field (STD/LTD).
• Disability Management Certificate/Diploma preferred.
• Bachelor's degree in Psychology, Social Work, Kinesiology or related field.
• Knowledge of disability management principles and processes.
• Familiarity with medical terminology.
• Ability to follow policies and guidelines.
• Strong analytical and detail-oriented skills.
• Excellent verbal and written communication and conflict resolution.

Responsibilities

• Adjudication and case management of short-term and long-term disability absences.
• Facilitate recovery and return-to-work for employees on medical leave.
• Conduct functionally focused telephone interviews with employees.
• Document case files and communicate with team members.
• Liaise with employees, insurers, employers and health care professionals.
• Develop and monitor return-to-work plans.
• Coordinate referrals to health resources.
