Job summary

The Korak Global Healthcare Services LLC Assessor Reviewer conducts assessments for persons with disabilities, primarily in person with occasional virtual sessions, to determine needed services in the community.

Assessments are submitted within 1-2 days after interviews for QA before client submission; candidates must have a health-care related degree and SIS certification, with at least 2 years’ experience in ID/DD settings, and reside in one of Maryland's listed counties.

Qualifications

Degree in health care or related field required.
Must pass and maintain SIS certification (Telligen will provide).
Minimum of 2 years of experience working with persons with intellectual and developmental disabilities.
Reliable transportation and proficiency in written and spoken English are essential.

Responsibilities

Performs face-to-face and/or virtual assessments of persons with disabilities.
Some contracts may require collaborating with healthcare providers and ancillary service providers to create or validate the appropriate treatment plan.
Documents assessment results for the individual member status or negotiates service plan as needed.
Provides ongoing education. May communicate with all health care providers to ensure appropriateness of care.
