Job summary

Ohio District 5 Agency On Aging is seeking a dedicated professional to perform assessments and screenings to link individuals with needed services. The role requires building connections with individuals, caregivers, and community agencies while navigating program specifications and timelines.

Responsibilities include conducting Level of Care and Comprehensive assessments, documenting thoroughly, and coordinating services across funding sources.

Qualifications

• Must be licensed to practice in Ohio (RN, LPN, or social worker).
• Strong verbal and written communication skills.
• Ability to work from home and travel as needed.
• Proficient with Microsoft Word and Excel.

Responsibilities

• Conducts phone calls and in-person contacts to assess needs per program specs and time frames.
• Assists individuals in accessing necessary services regardless of funding source.
• Establishes initial service needs and ensures no service duplication.
• Maintains documentation per agency policies and HIPAA requirements.
