Job summary

Axia Care Center in Orange, NJ, is seeking an experienced Director of Social Services to lead the Social Services Department in a skilled-nursing setting.

The ideal candidate will have at least two years of relevant social services experience, a NJ LSW or CSW, and hands-on MDS and POLST expertise, with a proven track record in care plans, discharge planning, and regulatory compliance, while supervising staff.

Qualifications

• At least two years of recent social services experience in skilled nursing or long-term-care.
• Active New Jersey LSW or CSW.
• Hands-on experience with MDS documentation.
• Hands-on experience completing and managing POLST forms.
• Experience investigating, documenting, and resolving resident and family grievances.
• Experience with care plans, discharge planning, advance directives, resident rights, and regulatory compliance.
• Previous leadership or supervisory experience preferred.

Responsibilities

• Direct the daily operations of the Social Services Department.
• Complete required MDS documentation accurately and on time.
• Manage POLST forms, advance directives, and healthcare decision-making documentation.
• Investigate and follow up on resident and family grievances.
• Participate in care-plan and interdisciplinary team meetings.
• Coordinate safe discharge planning and community services.
• Address psychosocial, behavioral-health, and resident-rights concerns.
• Maintain compliance with federal and New Jersey skilled-nursing regulations.
• Supervise Social Services staff and ensure timely documentation.

SKILLED-NURSING EXPERIENCE IS REQUIRED

Candidates must have:
