Career Strategies is seeking an experienced Social Services Director for a Long-Term Care and Subacute Facility in New Jersey. We are seeking an experienced and compassionate Social Services Director to lead the Social Services Department in a long-term care and subacute community. This role reports directly to the Administrator and plays a vital part in resident advocacy, regulatory compliance, and interdisciplinary collaboration.

Oversee all social services functions, including psychosocial assessments, care planning, and discharge planning
Maintain and audit POLST forms and coordinate weekly resident visits with the psychology provider
Participate in weekly Utilization Review (UR) meetings and manage weekly discharge planning updates
Ensure completion and distribution of the annual Resident Rights mailing
Maintain compliance with PASRR (Level I & II), grievance logs, and all applicable regulations
Complete MDS 3.0 documentation (Sections Q and C) and ongoing progress notes
Collaborate in interdisciplinary care conferences and support resident transitions of care
Provide coverage support for Admissions, including census awareness and insurance verification
Supervise and support one Social Worker, including hiring and performance management in collaboration with Administration and HR
Required: BSW, MSW, or state-recognized Certified Social Worker (CSW)
Experience: Prior experience in a Skilled Nursing Facility (SNF) or Long-Term Care setting
Strong knowledge of regulatory compliance, resident rights, and interdisciplinary care planning
Collaborative, self-directed leadership style with excellent communication skills
