Qualifications

• One year of supervised social work experience in a health care setting with individuals.
• One year of long-term care experience.

Responsibilities

• Identify residents’ psychosocial, mental, and emotional needs.
• Complete Social History and Psychosocial assessment of residents.
• Work with an interdisciplinary team to promote and protect residents’ rights.
• Prevent and address resident abuse as mandated by law and licensure.

Social Worker Director Responsibilities

Social Worker Director License Requirements

• Must have an active/ valid LSW or CSW
