Job summary

Ascension is seeking a leader to develop and implement policy and procedures for Population Health services in Baltimore, MD. You will collaborate with insurance and clinic partners to advance population health programs and service area goals while managing staffing and training initiatives.

The role requires leadership experience, understanding of healthcare policy, and budget oversight to ensure compliant operations within allocated funds.

Qualifications

• Must have a high school diploma or GED with 5 years of applicable experience, including 2 years in leadership/management.
• Associate's degree or Bachelor's degree with 3 years of applicable experience, including 2 years in leadership/management.
• Knowledge of Maryland's Total Cost of Care (TCOC) reimbursement model and value-based contracting is preferred.

Responsibilities

• Develop and lead the implementation of policy, procedures and initiatives for assigned services.
• Collaborate with insurance partners and clinic/hospital partners to develop Population Health programs.
• Develop service area goals and standards aligned with medical, administrative, legal and ethical requirements.
• Plan, organize and direct staffing activities including hiring, orientation, evaluation and continuing education initiatives.
• Prepare service area budgets and ensure the department operates within allocated funds.

How you’ll make an impact in this role

Education

What minimum requirements you’ll need

• High School diploma equivalency with 5 years of applicable cumulative job specific experience required, with 2 of those years being in leadership/management OR Associate's degree/Bachelor's degree with 3 years of applicable cumulative job specific experience required, with 2 of those years being in leadership/management.

What Additional Preferences We're Seeking

• Minimum of 2 years of leadership experience (leading people, teams, or departments)
• Master's degree required
• Prior experience with Maryland's Total Cost of Care (TCOC) reimbursement model and value-based contracting
