Job summary

Campbell Street in Des Moines, IA 50314 is seeking a responsible Social Service leader to oversee the department, ensuring residents’ biopsychosocial needs are met in compliance with federal, state, and local standards.

You will direct admissions and discharges, develop care plans with input from residents and families, participate in an interdisciplinary team, document thoroughly, and support discharge planning and budget processes while upholding resident rights.

Qualifications

• Minimum two years of supervisory experience in long-term care.
• Certification/licensure as required by state and federal regulations.

Responsibilities

• Plans and directs a comprehensive social services program for residents and families.
• Coordinates admissions and discharges to maintain census.
• Participates in an interdisciplinary team to better serve residents.
• Provides therapeutic intervention to help residents cope with illness and loss.
• Maintains written documentation in the medical record per policy and guidelines.
• Performs initial biopsychosocial assessments with ongoing updates.
• Develops social service components of the care plan with resident/family input.
• Acts as liaison between residents, families, outside agencies, and Administrator.
• Conducts family group meetings as needed (admissions, support groups).
• Completes required forms and reports under Administrator guidance.
• Facilitates discharge planning and organizes care transitions.
• Participates in departmental budget planning and monthly reporting.
