Job summary

The role includes discharge planning, resident and family assessment, and coordinating with community agencies to meet needs. Prior leadership and appropriate social work credentials are required.

Qualifications

• Must hold LCSW, BSW, or MSW credentials.

Responsibilities

• Assist in planning, developing, organizing, implementing, evaluating, and directing the social service programs.
• Interview resident/families to obtain social history.
• Discharge planning and development of social care plans.
• Coordinate social service activities with other departments and community agencies.
• Review department policies at least annually and recommend changes.
• In the absence of the Director, assume authority and responsibility of directing the social service department.
