Job summary

Responsibilities

• Oversee development and evaluation of the Social Services Department to support residents’ well-being.
• Complete and direct the social services component of the comprehensive assessment.
• Coordinate preadmission and interdisciplinary evaluations for new residents.
• Ensure documentation of social services in each resident’s medical record.
• Lead QA procedures and adapt them to improve care quality.
• Facilitate Advance Care Planning and review Advance Directives with residents.
• Address grievances and report resolutions; participate in QAPI.
