Qualifications

• Must be able to communicate in English, both verbally and in writing.
• Must be capable of performing the essential functions of the job.
• Knowledge of state, federal and local regulations related to long-term care.
• Minimum 1 year of experience in Social Services.

Responsibilities

• Monitor residents' mental and psycho-social needs and provide services.
• Direct the facility's social services operations with attention to age-specific development factors.
• Participate in pre-admission screening and provide information during admissions.
• Coordinate and review behavior charting for effective care plans.
• Provide therapeutic counseling to residents and families to meet psychosocial needs.
