Qualifications

• Experience with initial psychosocial assessments and resident follow-up is preferred.

Responsibilities

• Provide social services to residents and families under the direction of the Social Services Director.
• Meet new residents at admission and conduct initial psychosocial assessments, with follow-up as needed.
• Assist with arranging services and resources (Medicaid, Medicare, SSI).
• Coordinate scheduling of dental, eye, podiatry, and audiology visits; support families in time of loss.
