Sanford Health & Rehabilitation is seeking an experienced Social Worker to join our team. You will work alongside fellow caregivers to provide high quality patient care.

Responsibilities

• Assess nature of patients' situations by interviewing and reviewing personal history.
• Develop and execute individual treatment plans/care plans.
• Determine appropriate milestones to gauge patient progress.
• Communicate patient updates to all relevant parties.
• Facilitate referrals to other healthcare professionals and programs as needed.
• Maintain accurate documentation
