Epic Healthcare is actively seeking a Director of Social Services in Philadelphia to lead the social work program for adult and geriatric residents in a long-term care setting. The role emphasizes strategic planning, program development, and collaboration with clinical teams to ensure high-quality resident social services.

The position requires prior experience as a Director of Social Work in a large LTC facility, with a BSW/MSW or related bachelor’s degree, and at least one year in social work.

• Experience as a Director of Social Work in a large LTC setting preferred.
• BSW or MSW preferred, or Bachelor’s degree in related field.
• At least one year experience in Social Work field required.

Responsabilidades

• Demonstrates knowledge of age-specific developmental factors for adult and geriatric residents in planning social services programs.
• Plans social services programs for residents with needs: therapeutic, neurological and dementia, dialysis, IV therapy, and infectious diseases if applicable.
• Promptly follows-ups on admission inquiries, evaluates and assesses the needs of prospective residents to determine appropriateness of facility placement.

Duties and Responsibilities:

• Demonstrates knowledge of age specific developmental factors specific to adult and geriatric residents (i.e. physical, cognitive, and socialization factors) in planning social services programs.


• Demonstrates knowledge, skills, and techniques necessary to plan social services programs for residents with the following needs: therapeutic, neurological and dementia, dialysis, IV therapy, and infectious diseases if applicable.


• Promptly follows-up on admission inquiries, evaluates and assesses the needs of prospective residents to determine appropriateness of facility placement

Qualifications:

• Experience as a Director of Social Work in a large LTC setting preferred
