Job summary

Heritage Manor Care in California is seeking a dedicated Director of Social Services to lead the resident psychosocial program, oversee admissions and discharge procedures, and ensure holistic well-being for residents.

You will supervise social services staff, participate in Interdisciplinary Team meetings, and coordinate care plans while maintaining regulatory compliance and high-quality outcomes for residents and families.

Qualifications

• Bachelor's degree in Social Work, Human Services, Sociology, Education, Psychology, Rehabilitation/Counseling or related field.
• Minimum one year of experience in skilled nursing setting.
• SSD Certification; management experience preferred.

Responsibilities

• Coordinate admissions and discharge procedures.
• Attend Interdisciplinary Team and Unit Care conferences; supervise social services implementation and documentation.
• Interview new residents and assess psychosocial condition.
• Participate in Interdisciplinary Team Meetings; provide information on residents' psychosocial status and discharge prospects.
• Chart social service information in residents' charts; oversee psychoactive committee review with Nursing Director.
• Assist with discharge planning for homeward-bound residents; provide counseling referrals to outside agencies as needed.

As The Director Of Social Services You Will

Be a friendly and compassionate individual that will provide for the physical, psychological, and social needs of the residents of the facility, motivating residents to make healthy adjustments to the skilled nursing center. In this role, you will assume administrative authority, responsibility and accountability to provide medically-related social services. You will implement social service interventions that achieve treatment goals, address resident needs, link social supports, physical care and physical environment to enhance our resident's quality of life.

Duties

• The Director of Social Services coordinates admissions and discharge procedures.
• The Director of Social Services attends Interdisciplinary Team and Unit Care conferences, assesses residents for psychosocial groups, and supervises implementation and documentation.
• The DSS interview new residents and assess the resident's psycho-social condition.
• The DSS participates in Interdisciplinary Team Meetings and Care Conference providing information on individual resident's psycho-social condition and prospects for discharge or movement within the facility.
• The DSS charts relevant social service information into the resident's chart.
• The Social Services Director will oversee and co-manage with the Director of Nursing the psychoactive committee review.
• The DSS discusses problems encountered by residents and assists with the implementation of solutions.
• This position conducts follow-up procedures to ensure resident satisfaction.
• The DSS notes roommate problems and works toward resolution.
• This position makes recommendation as to whether a resident can function effectively at home or at other levels within the facility.
• The DSS assists with discharge planning for homeward bound residents.
• The Director of Social Service's attends in-service programs offered by the Nursing Home and may be requested to deliver in-services pertaining to Social Service functions.
• The Director of Social Services makes all counseling decisions and referrals to outside agencies on a regular basis.
• The Director of Social Services maintains contact with staff from all Departments of the Nursing Home in attempting to remedy the diverse problems of Nursing Home residents.
• The DSS maintains contact on a continuing basis with the Director and Assistant Director of Nursing to exchange information on the status of individual residents.

Requirements

Must have a Bachelor's degree in Social Work Human Services, Sociology, Education, Psychology, Rehabilitation/Counseling or related field required; Master's degree a plus.

Minimum one year of experience working in skilled nursing required and Management Experience.
