Job summary

Appone is seeking a Director of Social Services for a full-time role at Cottage Crest Post-Acute in California. You will lead social services for residents, coordinate psychosocial assessments, and collaborate with nursing leadership to enhance quality of life and care.

The position requires a Bachelor's in Social Work or related field, SSD certification, and at least one year of skilled nursing experience with management responsibilities.

Responsibilities

• Interview new residents and assess the resident's psychosocial condition; participate in Interdisciplinary Team Meetings providing information on psychosocial status and discharge prospects.
• Oversee and co-manage with the Director of Nursing the psychoactive committee review.
• Discuss problems encountered by residents and assist with solutions; follow-up to ensure resident satisfaction and assist with discharge planning.
• Maintain contact with staff from all departments to address resident needs and exchange information about status.

As the Director of Social Services you will:

Be a friendly and compassionate individual that will provide for the physical, psychological, and social needs of the residents of the facility, motivating residents to make healthy adjustments to the skilled nursing center. In this role, you will assume administrative authority, responsibility and accountability to provide medically-related social services. You will implement social service interventions that achieve treatment goals, address resident needs, link social supports, physical care and physical environment to enhance our resident's quality of life.

Duties:

• The Director of Social Services coordinates admissions and discharge procedures.
• The Director of Social Services attends Interdisciplinary Team and Unit Care conferences, assesses residents for psychosocial groups, and supervises implementation and documentation.
• The DSS interview new residents and assess the resident's psycho-social condition. The DSS participates in Interdisciplinary Team Meetings and Care Conference providing information on individual resident's psycho-social condition and prospects for discharge or movement within the facility. The DSS charts relevant social service information into the resident's chart.
• The Social Services Director will oversee and co-manage with the Director of Nursing the psychoactive committee review.
• The DSS discusses problems encountered by residents and assists with the implementation of solutions. This position conducts follow-up procedures to ensure resident satisfaction. The DSS notes roommate problems and works toward resolution. This position makes recommendation as to whether a resident can function effectively at home or at other levels within the facility. The DSS assists with discharge planning for homeward bound residents.
• The Director of Social Service's attends in-service programs offered by the Nursing Home and may be requested to deliver in-services pertaining to Social Service functions.
• The Director of Social Services makes all counseling decisions and referrals to outside agencies on a regular basis.
• The Director of Social Services maintains contact with staff from all Departments of the Nursing Home in attempting to remedy the diverse problems of Nursing Home residents. The DSS maintains contact on a continuing basis with the Director and Assistant Director of Nursing to exchange information on the status of individual residents.

• Must have a Bachelor's degree in Social Work Human Services, Sociology, Education, Psychology, Rehabilitation/Counseling or related field required; Master's degree a plus.
• Minimum one year of experience working in skilled nursing required and Management Experience.
• SSD Certification
