Job summary

Champion Rehabilitation and Nursing Center in Brockton, MA is seeking a qualified Social Worker or RN with social work experience to support residents and families.

You will develop care plans, coordinate admissions, support discharge planning, and participate in interdisciplinary care meetings with a focus on social and emotional needs.

Qualifications

• RN, BSW or MSW preferred, or Bachelor's degree in related field
• At least one year experience in Social Work field required
• Experience in long term care and/or sub-acute setting preferred

Responsibilities

• Maintaining and improving residents' ability to control daily physical needs and psychosocial needs
• Identifying individual social and emotional needs
• Developing and maintaining individualized care plans
• Participating in admission planning or transfers to another facility
• Participating in interdisciplinary care plan meetings for social needs
• Educating residents on documentation and discharge planning
• Coordinating post-discharge services and home care when needed

• Maintaining and/or improving resident’s ability to control everyday physical needs and mental/psychosocial needs

• Identifying individual social and emotional needs

• Developing and maintaining individualized care plans

• Participating in the planning of the resident’s admission, community re-entry or transfer to another facility

• Participating in interdisciplinary care plan meetings to ensure treatment of the social and emotional needs of the resident

• Work with the finance team to ensure resident is educated and the proper documents are in place for the resident during their stay

• Work with the resident on discharge and get necessary DME or home care in place so that the resident is safe when they return to the community

• Demonstrates knowledge of age specific developmental factors specific to adult and geriatric residents (i.e. physical, cognitive, and socialization factors) in planning social services programs.

• Demonstrates knowledge, skills, and techniques necessary to plan social services programs for residents with the following needs: therapeutic, neurological and dementia, dialysis, IV therapy, and infectious diseases if applicable.

• Promptly follows-up on admission inquiries, evaluates and assesses the needs of prospective residents to determine appropriateness of facility placement
