Job summary

Champion Rehabilitation and Nursing Center is seeking a dedicated CNA to assist residents with daily living activities, hygiene, nutrition and mobility. You will document intake, observe health changes and collaborate with nursing staff to ensure high-quality care in a compassionate environment.

Required CNA license with 1 year of experience preferred. The role emphasizes confidentiality, adherence to policies, and respectful resident interactions in a family-friendly setting.

Qualifications

• CNA license is required.
• 1 year CNA experience preferred.

Responsibilities

• Assist Residents with daily living activities including meals, turning, ambulating and providing fluids and nourishment.
• Help Residents with personal hygiene.
• Observe Residents and report health issues to the supervising nurse.
• Document Resident food and fluid intake.
• Assist Residents with feeding and nutritional supplements.
• Make beds and keep living areas clean and tidy.
• Transport Residents within the facility.
• Assist nurses and other staff as needed.
• Adhere to professional standards and follow facility policies and procedures.
• Abide by federal, state and local requirements and standards.
• Maintain the confidentiality of all Residents and Facility data.

Duties and Responsibilities:

• Assist Residents with daily living activities, to include, delivering meals, turning and ambulating residents and providing plenty of fluids and nourishment meals throughout scheduled shift

• Help Residents with personal hygiene

• Observe Residents and report any health issues to the supervising nurse

• Document Resident food and fluid intake

• Assist Residents with feeding and serving nutritional supplements

• Making the Resident’s beds and keeping their living areas clean and tidy

• Transport Residents within the facility

• Assist nurses and other staff as needed

• Adhere to professional standards and follow all facility policies and procedures

• Abide by all federal, state and local requirements and standards

• Maintain the confidentiality of all Residents and Facility data

Experience:

• CNA: 1 year (Preferred)
