Job summary

Champion Rehabilitation and Nursing Center in Brockton, MA is seeking a qualified social worker to support residents through personalized care planning and community re-entry. You will collaborate with an interdisciplinary team, assess social and emotional needs, and coordinate admissions, discharges, and home care arrangements to ensure safe transitions.

Qualified candidates have RN/BSW/MSW preferred or Bachelor’s in related field and at least one year of social work experience in long-term

Qualifications

• RN, BSW or MSW preferred, or Bachelors degree in related field
• At least one year experience in Social Work field required
• Experience in long term care and/or sub-acute setting preferred

Responsibilities

• Maintaining and/or improving resident’s ability to control everyday physical needs and mental/psychosocial needs
• Identifying individual social and emotional needs
• Developing and maintaining individualized care plans
• Participating in the planning of the resident’s admission, community re-entry or transfer to another facility
• Participating in interdisciplinary care plan meetings to ensure treatment of the social and emotional needs of the resident
• Work with the resident on discharge and get necessary DME or home care in place so that the resident is safe when they return to the community
• Demonstrates knowledge of age specific developmental factors specific to adult and geriatric residents (i.e. physical, cognitive, and socialization factors) in planning social services programs.
• Demonstrates knowledge, skills, and techniques necessary to plan social services programs for residents with the following needs: therapeutic, neurological and dementia, dialysis, IV therapy, and infectious diseases if applicable.

Duties and Responsibilities:

• Work with the finance team to ensure resident is educated and the proper documents are in place for the resident during their stay
• Work with the resident on discharge and get necessary DME or home care in place so that the resident is safe when they return to the community
• Demonstrates knowledge of age specific developmental factors specific to adult and geriatric residents (i.e. physical, cognitive, and socialization factors) in planning social services programs.
• Demonstrates knowledge, skills, and techniques necessary to plan social services programs for residents with the following needs: therapeutic, neurological and dementia, dialysis, IV therapy, and infectious diseases if applicable.
• Promptly follows-up on admission inquiries, evaluates and assesses the needs of prospective residents to determine appropriateness of facility placement
