Job summary
Dorchester Seniors Inc. is seeking a Director of Human Services & Care Coordination to lead its Intervention & Stabilization Services across Dorchester County. The role focuses on case management, care coordination, resource navigation, rural outreach, and staff supervision.
The ideal candidate will have a BA in social work or related field, 5+ years in human services, and proven leadership. Travel throughout Dorchester County is required, with a provided DSI vehicle.
Qualifications
- Bachelor's degree in social work or related field.
- Minimum five years in human services, aging services, or care coordination.
- Two+ years supervisory/leadership experience.
- Ability to supervise staff and manage performance.
- Experience with program development and community partnerships.
Responsibilities
- Provide strategic and operational leadership for Intervention & Stabilization Services.
- Oversee case management, care coordination, and rural outreach.
- Establish standards for referrals, intake, and documentation.
- Ensure compliance with confidentiality and scope‑of‑practice requirements.
- Lead staff supervision and performance management.
- Develop partnerships with community organizations to expand services.
