Job summary

Dubois Continuum of Care Community in DuBois, Pennsylvania seeks a dynamic Social Services Director with a strong background in senior care or community-based organizations. You will plan, organize, develop, administer, and evaluate the social services program and oversee the department and staff.

The role requires leadership in coordinating social services, policy development, and collaboration with interdisciplinary teams to support residents' well-being.

Qualifications

• Minimum 2 years' experience in a nursing home or assisted living/personal care setting.
• At least 2 years in a management supervisory role.
• Bachelor's degree in social work or related field; LSW/MSW preferred.

Responsibilities

• Supervise Social Services staff, including performance evaluations and personnel matters.
• Hold individual and department staff meetings regularly.
• Develop and maintain policies and procedures for the Social Service department, ensuring compliance with regulations.
• Facilitate internal programs that enhance residents' psycho-social functioning (Resident Council, QAPI teams, family groups).
• Provide supportive intervention, counseling with residents to resolve conflicts, and education.
• Serve as liaison to hospice providers and complete MDS assessments, documentation, and care planning as required.
• Participate in interdisciplinary team meetings, providing professional feedback and education.
• Prepare and maintain the department budget and monitor expenditures.
• Complete daily, weekly, monthly, quarterly, and annual reporting requirements.
• Maintain a safe work environment and adhere to facility policies and procedures.
