Job summary

Dubois Continuum of Care Community in DuBois, PA is seeking a dynamic Social Services Director to lead resident-focused services, develop programs, and oversee staff and operations of the Social Services department.

The ideal candidate will have a bachelor’s degree in social work or related field (LSW/MSW preferred) and at least two years in a supervisory role within a nursing home or assisted living setting, with strong regulatory compliance and budget management.

Qualifications

• 2+ years of experience in a nursing home or assisted living/personal care setting
• Minimum 2 years in a management supervisory role
• Bachelor's degree in social work or related field required; LSW/MSW preferred
• High school diploma or GED required

Responsibilities

• Supervise Social Services staff, including performance evaluations and personnel matters
• Hold regular individual and department staff meetings
• Develop and maintain policies and procedures for the Social Services department
• Coordinate internal programs to support residents' psycho-social functioning
• Provide supportive counseling and guidance to residents
• Liaise with hospice providers; complete MDS assessments and care planning
• Participate in interdisciplinary team meetings and provide education
• Prepare and monitor the department budget and expenditures
• Complete daily, weekly, monthly, quarterly, and annual reporting
• Maintain a safe work environment and adhere to policies
