Job summary

CHANGE, Inc. in Weirton, WV, is hiring a Director of Quality Care to lead the quality management program and coordinate with clinical leadership. The role requires active RN licensure in OH and WV and a strong background in quality improvement.

You will train staff, ensure PCMH and HRSA reporting, oversee multiple quality platforms, and collaborate with care teams to drive continual quality improvement initiatives. The position is full-time, Monday through Friday, onsite.

Qualifications

• Active RN licensure in OH and WV.
• Quality Management experience preferred.
• Excellent computer skills; proficient with Excel and Word.
• Strong organization, communication, interpersonal and multi-tasking abilities.
• Ability to work with diverse patients and staff and promote a positive image.
• Must maintain a valid driver’s license.
• Pass a background check and drug screening.

Responsibilities

• Oversee the QM program with Clinical Site Managers, CMO, Director of Compliance and Risk, and care teams.
• Conduct annual training of clinical staff and HRSA reporting.
• Lead monthly and quarterly meetings with staff and insurance companies.
• Maintain multiple quality platforms.
• Supervise the Chronic Care Coordinator, Clinical Health Information Specialist, and Clinical Quality Auditor.
• Oversee Quality Initiatives for Community Action Programs.

Position Details

• Job Title: Director of Quality Care
• Location: Weirton
• Work Schedule: Monday – Friday

Responsibilities

• Job duties would include but are not limited to;
• Working directly with management and being responsible for oversite of the QM program, which includes working closely with Clinical Site Mangers, CMO, Director of Compliance and Risk, and Care Teams for continual quality improvement initiatives
• Conduct annual training of Clinical staff, maintain PCMH certification and annual HRSA reporting including provider education on quality (peer reviews, targeted chart reviews)
• Conduct meetings on monthly and quarterly intervals for various staff and insurance companies.
• Maintain multiple quality platforms
• Oversight of the Chronic Care Coordinator, Clinical Health Information Specialist, and Clinical Quality Auditor
• Oversee Quality Initiatives for Community Action Programs

Qualifications

• Active RN Licensure in OH and WV
• Quality Management experience preferred.
• Must have excellent Computer skills with knowledge of Microsoft Excel and Word, must have the ability to grasp new technology quickly.
