Job summary

Colwyn Group in Avon, CT is seeking an experienced Director of Nursing Services to lead the nursing department of a 120-bed SNF and rehabilitation center. This hands-on leader will drive clinical excellence, mentor staff, ensure compliance with DPH and CMS standards, and uphold resident-centered care.

The role requires an active CT RN license, BLS, a BSN preferred, and 3+ years in SNF or geriatric long-term care with 1–2+ years in leadership.

Qualifications

• RN license active in Connecticut (or multi-state eligible for CT reciprocity)
• BLS/CPR certification current
• 3+ years of progressive nursing experience in SNF/subacute/geriatric long-term care
• Minimum 1–2+ years in a nursing leadership role (DNS/DON/ADNS)
• Deep knowledge of CMS regulations, CT DPH mandates, Title 18/19 guidelines, and QAPI methodologies

Responsibilities

• Direct overall nursing department operations across all shifts and maintain round-the-clock standards.
• Mentor and evaluate unit managers, charge nurses, and CNAs.
• Oversee staffing, scheduling, and resource allocation to optimize resident care and efficiency.
• Maintain floor presence through clinical rounding and ensure resident/family engagement.
• Ensure survey readiness and compliance with DPH and CMS regulations.
• Lead QAPI committee and monitor infection control and fall prevention.
• Oversee documentation, assessments, MDS alignment, and SOPs.
• Investigate incidents and develop corrective action plans (POCs).
• Collaborate with Facility Administrator, Medical Director, and Therapy leadership.
• Advocate for residents and provide clear communication with families.
• Foster a transparent, accountable work environment for clinical staff.
