Job summary

Qualifications

• MSW in good standing required.
• Strong leadership and communication skills.
• Experience in a skilled nursing facility and with care planning.

Responsibilities

• Plan, develop, and implement social service programs for residents.
• Liaise with residents, families, and staff for care coordination.
• Assist with understanding illness and placement in long-term care.
• Support care planning, discharge planning, and transitions of care.
• Maintain compliance with regulations and document assessments and plans.
• Supervise and mentor social services staff.
