Job summary

Avir Trilogy is seeking a Director of Nursing to join our team in Texas. This role leads from the floor, planning, organizing, and directing the day-to-day work of the Nursing Services Department to keep us survey-ready and compliant with regulations.

The ideal candidate is an RN with supervisory experience and the judgment to drive resident outcomes. You'll supervise the nursing management team, participate in quality programs, and ensure staffing and safety standards while delivering

Qualifications

  • Current, unencumbered Registered Nurse (RN) license in the state of Texas.
  • Minimum of one year of supervisory experience in a hospital, skilled nursing facility, or related healthcare setting.
  • Working knowledge of long-term care operational standards under the Federal Register, Conditions of Participation, and state regulations.
  • Ability to work independently, exercise sound judgment, and remain calm under pressure.
  • Patience, tact, and a positive attitude with residents, families, and staff.
  • Comfort working with residents who are ill, disabled, elderly, or at times upset.
  • An active role requiring standing, walking, bending, kneeling, and stooping throughout the day.

Responsibilities

  • Direct nursing operations to meet federal, state, and regulatory care standards, and keep the facility survey-ready year round.
  • Lead the nursing management team and build ownership of quality goals across the department.
  • Run continuous quality assessment and improvement of nursing care delivery, and keep the Administrator current on department activity and issues.
  • Help develop and implement plans of action that support each resident's comprehensive assessment and care plan.
  • Make daily rounds to confirm care standards are being met and to monitor safety, reporting unsafe conditions and all incidents immediately.
  • Audit charts for accuracy and completeness, identify problem areas, and implement corrective action.
  • Maintain appropriate staffing levels through work assignments, schedule adjustments, and shift approvals.
  • Serve on facility committees including Infection Control, Quality Assurance, and Utilization Review.
  • Participate in the Infection Control program, monitoring hand hygiene, isolation technique, dressing changes, linen handling, and medication pass at least monthly.
  • Review resident complaints and grievances and document the actions taken to resolve them.
  • Monitor weekly skin assessments and bath sheets, and complete the weekly wound report.
  • Participate in the nursing on-call rotation.
