All Seniors Foundation in Los Angeles, CA seeks a Director of Nursing to provide clinical leadership for the home health program, supervise RNs, LVNs, HHAs, and therapy staff, and ensure CMS compliance.

Qualifications

• CA RN license + BSN required; MSN preferred.
• 5+ years home health nursing including 2+ years in management.
• Familiarity with OASIS, CMS regulations, Joint Commission, and CA DPH home health rules.
• Strong leadership, communication, and conflict-resolution skills.
• COS-C/CPHQ certification a plus (preferred)

Responsabilités

• Lead the QAPI program: audits, performance improvement, Star Ratings, HHCAHPS.

• Represent the agency in state surveys, accreditation reviews, and payer audits.

Role Overview:

As Director of Nursing, you will provide clinical leadership for our entire home health program — supervising nursing staff, ensuring CMS compliance, leading QAPI, and shaping clinical strategy. You report directly to the CEO and partner with the Medical Director on clinical quality.

Key Responsibilities:

• Provide clinical supervision and leadership to RNs, LVNs, HHAs, and therapy staff (40+ clinicians)
• Ensure compliance with CMS Conditions of Participation, CA Title 22, and Joint Commission standards
• Lead the QAPI program: chart audits, performance improvement, Star Ratings, HHCAHPS
• Develop and update clinical policies, procedures, and competenciesManage clinical hiring, onboarding, performance evaluation, and progressive discipline
• Represent the agency in state surveys, accreditation reviews, and payer audits

Qualifications:

• Current CA RN license + BSN required; MSN strongly preferred
• 5+ years home health nursing including 2+ years in management
• Familiarity with OASIS, CMS regulations, Joint Commission, and CA DPH home health rules
• Strong leadership, communication, and conflict-resolution skills
• COS-C, CHPN, or CPHQ certification a plus
