Job summary

All Seniors Foundation in Los Angeles, CA seeks a Director of Nursing to provide clinical leadership for the home health program, supervise RNs, LVNs, HHAs, and therapy staff, and ensure CMS compliance.

You will lead QAPI, develop policies, manage hiring and performance, and collaborate with the CEO and Medical Director. The role offers executive visibility, a competitive salary, and comprehensive benefits.

Qualifications

• CA RN license + BSN required; MSN preferred.
• 5+ years home health nursing including 2+ years in management.
• Familiarity with OASIS, CMS regulations, Joint Commission, and CA DPH home health rules.
• Strong leadership, communication, and conflict-resolution skills.
• COS-C/CPHQ certification a plus (preferred)

Responsibilities

• Provide clinical supervision and leadership to 40+ clinicians (RNs, LVNs, HHAs, therapy staff).
• Ensure CMS Conditions of Participation compliance and CA Title 22.
• Lead the QAPI program: audits, performance improvement, Star Ratings, HHCAHPS.
• Develop and update clinical policies, procedures, and competencies; manage hiring and performance.
• Represent the agency in state surveys, accreditation reviews, and payer audits.
