Job summary

American Home Health in Los Angeles is seeking a Director of Patient Care Services / Director of Nursing to lead our home health clinical team, ensure regulatory compliance, and drive quality outcomes that help patients heal safely in the place they love most.

The role requires an active California RN, BSN (MSN preferred), 5+ years of nursing leadership in home health, and expert knowledge of Medicare regulations, OASIS, and PDGM.

Qualifications

• Active RN license in California (BSN required; MSN preferred).
• 5+ years of progressive nursing leadership experience in home health.
• Deep knowledge of Medicare home health regulations, OASIS, and PDGM reimbursement.
• Proven track record in survey readiness, quality assurance, and performance improvement.
• Exceptional communication and relationship-building skills with staff, physicians, and referral partners.
• Certification in Home Health Nursing (e.g., COS-C, CHPN) or Nursing Administration is a plus.
• Experience with EMR systems used in home health (e.g., Homecare Homebase, PointClickCare) is a plus.

What you'll do:

Lead and manage all clinical operations

Supervise, mentor, and develop clinical managers and field staff, fostering a culture of collaboration and excellence

Ensure full compliance with Medicare, Medi-Cal, and state home health regulations, including Conditions of Participation
