Job summary

All Seniors Foundation in Los Angeles, CA seeks a Director of Nursing (DON) to provide clinical leadership for our home health program, supervise nursing staff, ensure CMS compliance, and drive QAPI and clinical strategy.

Reporting to the CEO, you will partner with the Medical Director on quality, develop policies, hire and evaluate clinicians, and represent the agency in surveys and audits.

Qualifications

• Current CA RN license with BSN required; MSN strongly preferred.
• 5+ years home health nursing including 2+ years in management.
• Familiarity with OASIS, CMS regulations, Joint Commission, and CA DPH home health rules.
• Strong leadership, communication, and conflict-resolution skills.
• COS-C, CHPN, or CPHQ certification a plus.

Responsibilities

• Provide clinical supervision and leadership to RNs, LVNs, HHAs, and therapy staff (40+ clinicians).
• Ensure CMS Conditions of Participation, CA Title 22, and Joint Commission standards compliance.
• Lead the QAPI program: chart audits, performance improvement, Star Ratings, HHCAHPS.
• Develop and update clinical policies, procedures, and competencies.
• Manage clinical hiring, onboarding, performance evaluation, and progressive discipline.
• Represent the agency in state surveys, accreditation reviews, and payer audits.

Role Overview

As Director of Nursing, you will provide clinical leadership for our entire home health program — supervising nursing staff, ensuring CMS compliance, leading QAPI, and shaping clinical strategy. You report directly to the CEO and partner with the Medical Director on clinical quality.
