Position Summary:

The DON is responsible for the coordination of healthcare needs, evaluation and improvement of program services, and the delivery of high-quality services through the nursing process. Ensure quality services are consistent with the philosophy of the Assisted Living Community and comply with applicable state laws and regulations. Conduct all responsibilities within the Scope of Practice dictated by the state, meeting Federal and State regulations as applicable. Provide personal care services, assistance, support, evaluation, and supervision to residents as needed. Candidates will have the ability to enrich residents’ lives by addressing their physical, intellectual, emotional, and social well-being.

Essential Job Responsibilities:
  1. Coordinate services to meet residents' health needs and promote wellness.
  2. Provide personal care, assistance, support, evaluation, and supervision to residents as needed.
  3. Perform pre-admission assessments of potential new residents.
  4. Meet with each new resident and complete a basic health assessment within one week of move-in; update service plans within 30 days.
  5. Complete 90-day medication reviews and 30-day health evaluation reviews for all residents.
  6. Develop assistance plans for new residents.
  7. Work with the team to develop and update service plans for residents.
  8. Coordinate assistance plans for at-risk residents, activate resources, and communicate with physicians and responsible parties.
  9. Ensure accountability for specialty units such as Dementia/Alzheimer’s units.
  10. Guide, evaluate, perform, and supervise resident assistance plans based on physician orders.
  11. Establish and maintain resident records according to state and federal guidelines.
  12. Review incident reports and ensure proper documentation.
  13. Provide or coordinate educational opportunities related to health issues (e.g., Alzheimer’s support groups).
  14. Establish medication setup systems to promote resident independence.
  15. Communicate changes in residents' conditions to relevant parties.
  16. Assume on-call responsibilities and address emergencies.
  17. Oversee Quality Improvement, Safety, and OSHA programs for the nursing department.
Education, Experience, and Other Requirements:
  • Nursing license in the state (RN preferred)
  • CPR certification
Knowledge, Skills, and Abilities Required:
  • Promote teamwork in providing services to residents.
  • Knowledge of OSHA-MSDS sheets and procedures.
  • Understanding of State and Federal Guidelines for Assisted Living and Dementia care.
  • Knowledge of the State Nurse Practice Act.
  • Compliance with HIPAA requirements.
  • Safe food handling procedures.
  • Ability to travel occasionally for meetings.
  • Organizational and prioritization skills with attention to detail.
  • Excellent assessment, interpersonal, and communication skills.
  • Ability to work within a multidisciplinary team.
  • Knowledge of nursing standards of practice for older adults.
Preferred Qualifications:
  • Previous memory care experience.
  • Experience in senior living.
  • Basic computer skills.
  • Knowledge of local referral resources.
  • Experience managing and directing staff.
  • Willingness to learn computer programs used in the facility.
