Job summary

Hollywood Premier seeks a Director of MDS in California to guide the development and transmission of resident assessments (MDS) and ensure compliant care planning.

The role requires an active CA RN license, CPR certification, and 2-4 years of direct patient care with MDS experience; supervision experience is a plus.

The incumbent will oversee initial assessments within 14 days of admission and coordinate with care teams to ensure measurable objectives and timely care plan updates.

Qualifications

• 2-4 years of direct patient care experience preferred; MDS experience required
• Nursing degree from accredited college or graduation from approved RN program
• Must possess current, unencumbered license to practice as an RN in California
• Current CPR certification

Responsibilities

• Conducts and coordinates the development and completion of resident assessments (MDS) in accordance with rules and guidelines
• Ensures care plans have measurable objectives and involve appropriate health professionals
• Ensures initial resident assessment is completed within fourteen days of admission and signed by the assesse/author
• Interview residents for assessments and update care plans quarterly and upon significant condition changes
• Evaluate condition and data to determine need to amend admissions assessments
• Other tasks as assigned

Director of MDS Summary:

The Director of MDS conducts and coordinates the development, completion and transmission of the resident assessment in accordance with the requirements of this state and policies and goals of this facility.

Essential Duties and Responsibilities:

• Conducts and coordinates the development and completion of preliminary and comprehensive resident assessment (MDS) in accordance with current rules, regulations, and guidelines, including the implementation of CAAs and Triggers and Plan of Care.
• Ensures that the care plan includes measurable objectives and timetables to meet the resident's medical, nursing, mental and psychosocial needs, that appropriate health professionals are involved in the assessment, and that the schedule of activities is developed as required.
• Ensures that an initial resident assessment is completed within fourteen days of the resident's admission and that each portion of the assessment is signed and dated by the person completing that portion of the MDS.
• Conducts or coordinates the interviewing of each resident for the assessment.
• Helps develop and update a comprehensive resident assessment and care plan for each resident. Ensures that it is updated on a quarterly and annual basis and upon significant change in condition.
• Evaluates each resident's condition and pertinent medical data to determine any need to amend the admission assessment.
• Other tasks as assigned.

Qualifications:

• 2-4 years' experience providing direct care to multiple patients/residents preferred; previous experience in Skilled Nursing environment desirable; previous MDS experience required; supervisory experience preferred
• Nursing degree from accredited college or graduation from approved RN program
• Must possess current, unencumbered license to practice as an RN in California
• Current valid CPR certification
