Job summary

Palazzopostacute is seeking a Director of MDS to oversee development, completion and transmission of resident MDS assessments in compliance with state requirements and facility goals.

You will coordinate care plans ensuring measurable objectives, involve appropriate health professionals, and ensure timely assessments and updates. The role requires CA-licensed RN/LVN and CPR certification, with 2–4 years of direct care experience; RN preferred.

Qualifications

• 2-4 years' direct care experience across multiple patients/residents.
• Nursing degree from an accredited college or LVN program; RN preferred.
• Current, unencumbered California nursing license (LVN or RN).
• Current CPR certification.

Responsibilities

• Coordinates the development and completion of resident MDS assessments per state requirements.
• Ensures care plans have measurable objectives and involve appropriate health professionals.
• Initial resident assessment completed within 14 days of admission with signed portions.
• Interviews residents for assessments and maintains accurate documentation.
• Develops/updates comprehensive assessments and care plans on a quarterly/annual basis or with significant changes.
• Performs other tasks as assigned.

Director of MDS Summary:

The Director of MDS conducts and coordinates the development, completion and transmission of the resident assessment in accordance with the requirements of this state and policies and goals of this facility.

Essential Duties and Responsibilities:

• Conducts and coordinates the development and completion of preliminary and comprehensive resident assessment (MDS) in accordance with current rules, regulations, and guidelines, including the implementation of CAAs and Triggers and Plan of Care.
• Ensures that the care plan includes measurable objectives and timetables to meet the resident's medical, nursing, mental and psychosocial needs, that appropriate health professionals are involved in the assessment, and that the schedule of activities is developed as required.
• Ensures that an initial resident assessment is completed within fourteen days of the resident's admission and that each portion of the assessment is signed and dated by the person completing that portion of the MDS.
• Conducts or coordinates the interviewing of each resident for the assessment.
• Helps develop and update a comprehensive resident assessment and care plan for each resident. Ensures that it is updated on a quarterly and annual basis and upon significant change in condition.
• Evaluates each resident's condition and pertinent medical data to determine any need to amend the admission assessment.
• Other tasks as assigned.

Qualifications:

• 2-4 years' experience providing direct care to multiple patients/residents preferred; previous experience in Skilled Nursing environment desirable; previous MDS experience required; supervisory experience preferred
• Nursing degree from accredited college or graduation from approved LVN program, RN preferred
• Must possess current, unencumbered license to practice as an LVN or RN in California
• Current valid CPR certification
