Job summary

Granite Hills Hospital in Wisconsin is seeking a Director of Intake & Referral to lead the Assessment & Referral team, oversee clinical evaluations, crisis interventions, and referrals for patients across age groups. The role interfaces with medical staff and administration to ensure streamlined access and care coordination.

The position requires a master's degree in a relevant field and at least five years of psychiatric/mental health experience, with appropriate licensure.

Qualifications

• Master's degree in social work, psychology, mental health, or related field; or nursing.
• Minimum of five years of direct clinical psychiatric/mental health experience.

Responsibilities

• Direct a staff of Assessment & Referral Counselors in psychiatric assessments and crisis intervention.
• Interface with medical staff, department heads, and case managers to coordinate intake services.
• Monitor timely completion of HBIPS Abstraction for all admissions.

The Director of Intake & Referral is accountable for directing the Assessment and Referral activities at the facility. As a clinician, this position provides direction to a staff of Assessment & Referral Counselors in the provision of psychiatric assessment services, including clinical evaluations, crisis intervention, and referrals. This position interfaces closely with the medical staff, fellow department heads and administration, treatment team/external case managers/managed care organizations, functions as a liaison with other hospital intake departments. Monitors and assures timely completion of the Core Measures (HBIPS) Abstraction Tool for all admissions.

Qualifications

Master's Degree from an accredited college or university in social work, psychology, mental health, or related field OR graduation from an accredited program of professional nursing.

A minimum of five (5) years direct clinical experience in a psychiatric or mental health setting desirable.

R.N., L.C.S.W., L.M.S.W., LPC or applicable license.
