Job description

Nursing Home Director of Growth & Community Relations
SNF & ALF Experience Required

We are seeking a dynamic, well-connected, and results-driven Director of Growth & Community Relations to oversee growth and community engagement initiatives at our facilities.

This is a key leadership position for an individual who understands the senior healthcare landscape, knows how to build lasting relationships, and has a demonstrated ability to grow census, strengthen referral networks, and expand a community’s presence in the market.

The ideal candidate brings hands‑on experience in both Skilled Nursing (SNF) and Assisted Living (ALF) and understands how to navigate the relationships that drive occupancy and long‑term growth.

THE OPPORTUNITY

As the Director of Growth & Community Relations, you will serve as a strategic representative of our organization throughout the healthcare and business community.

You will work across multiple communities to identify opportunities, develop partnerships, strengthen existing relationships, and create a consistent pipeline of qualified referrals.

This role is ideal for a healthcare growth professional who is equally comfortable in the field building relationships, in the community representing our organization, and with leadership analyzing results and developing strategy.

WHAT YOU'LL LEAD
  • Develop and execute growth strategies across multiple SNF and ALF communities.

  • Build meaningful, long‑term relationships with hospitals, physicians, discharge planners, case managers, social workers, rehabilitation providers, home health, hospice, and other healthcare partners.

  • Expand and diversify each community's referral network.

  • Identify new opportunities within existing markets and develop strategies for entering new referral channels.

  • Drive sustainable census and occupancy growth through relationship development and strategic outreach.

  • Develop community partnerships that increase awareness, trust, and engagement.

  • Represent the organization at healthcare events, professional organizations, networking functions, and community initiatives.

  • Partner with Executive Directors, administrators, clinical leadership, and operations to align growth strategies with each community's capabilities and goals.

  • Evaluate market trends, competitor activity, referral patterns, and census performance to identify opportunities.

  • Establish measurable goals and accountability standards for growth and community relations activities.

  • Develop and mentor community relations professionals and create a culture focused on accountability and results.

  • Provide leadership with regular reporting, market intelligence, and recommendations for continued growth.

WHAT WE REQUIRE

SNF and ALF experience is required.

We are looking for a candidate with:

  • Proven experience driving growth within the senior healthcare industry.

  • Demonstrated success increasing census, occupancy, referrals, or market share.

  • Experience working with both Skilled Nursing and Assisted Living communities.

  • Strong existing relationships within the healthcare referral community.

  • Experience working with hospitals, physicians, case managers, discharge planners, rehabilitation organizations, home health, hospice, and other healthcare partners.

  • Experience supporting or leading growth initiatives across multiple communities.

  • Exceptional communication, presentation, negotiation, and relationship-building skills.

  • Strong business judgment and the ability to turn market opportunities into measurable results.

  • A high level of professionalism and the ability to represent multiple communities and leadership teams.

  • Willingness to travel between communities and spend significant time in the field.

WHO WILL THRIVE IN THIS ROLE

You are someone who:

  • Builds relationships before asking for business.

  • Knows the senior healthcare market and understands what drives referrals.

  • Has a strong network and isn't afraid to pick up the phone or walk into a hospital.

  • Can open doors and create relationships where none previously existed.

  • Understands that census growth is the result of trust, reputation, responsiveness, and execution.

  • Can develop strategy but is equally comfortable getting into the field and executing it.

  • Holds yourself accountable to measurable outcomes.

  • Can influence and motivate teams across multiple communities.

  • Brings energy, professionalism, and credibility to every interaction.

HOW SUCCESS WILL BE MEASURED

Your success will be reflected in measurable growth, including:

  • Increased census and occupancy

  • Growth in qualified referral relationships

  • Increased referral volume

  • Stronger hospital and healthcare partnerships

  • Improved referral conversion

  • Development of new referral channels

  • Increased community visibility and engagement

  • Consistent growth across multiple communities

  • Development of a high-performing community relations team

WHY THIS ROLE MATTERS

Every relationship you build has the potential to connect an individual or family with the care they need.

This position is about more than increasing census. It is about building trust in our communities, strengthening healthcare partnerships, and ensuring families and healthcare professionals know where they can turn when quality senior care is needed.

If you have the SNF and ALF experience, healthcare relationships, leadership ability, and proven track record of growth, we want to hear from you
