Indiana Fitness Works seeks a Director of Dementia Care to lead memory care programs in our facilities. You will oversee care plans, ensure regulatory compliance, and guide teams to deliver dignified, person-centered resident care.

Qualifications

• Must have degree in healthcare, mental health, or social services or be a licensed health facility administrator.
• At least one year of dementia or Alzheimer's care within the past five years.
• Proven leadership experience managing dementia/memory care teams in long-term care.
• Extensive assisted living with nursing home or hospice background.
• Solid knowledge of long-term care regulations and Medicare policies.
• Proficient with EMR/EHR for clinical documentation review.
• Strong medical terminology, physiology, and anatomy knowledge for aging populations.
• Ability to develop individualized care plans compliant with HIPAA standards.
• Supervisory experience in healthcare with nurse management focus.

Responsibilities

• Lead the design and execution of dementia care programs tailored to residents' needs.
• Supervise and mentor nursing staff, care coordinators, and support teams.
• Oversee development and review of individualized care plans with medical history.
• Ensure compliance with long-term care regulations, HIPAA, Medicare guidelines.
• Manage documentation reviews in EMR/EHR for accuracy and completeness.
• Facilitate staff training on memory care topics and documentation procedures.
• Collaborate with interdisciplinary teams to optimize outcomes and resources.
