Job summary

Indiana Fitness Works Inc. seeks a Director of Dementia Care to lead memory care programs for a new Pine Knoll facility addition.

You will oversee design and execution of dementia care strategies, mentor nursing staff, and develop individualized care plans that respect residents’ dignity and rights. You will ensure compliance with long-term care regulations, HIPAA, and Medicare guidelines, while managing EMR/EHR documentation and staff training.

Qualifications

• Must have earned a degree from and educational institution in a healthcare, mental health, or social service profession or be a licensed health facility administrator.
• Minimum of one year work experience with Dementia or Alzheimer's residents within the past five years.
• Proven leadership experience managing long-term care facility teams with a focus on dementia or memory care programs.
• Extensive assisted living experience coupled with nursing home or hospice care background.
• Strong understanding of long-term care regulations, Medicare policies, and regulatory compliance standards.
• Proficiency in EMR/EHR systems for clinical documentation review and management of medical records.
• In-depth knowledge of medical terminology, physiology, anatomy relevant to aging populations with cognitive impairments.
• Ability to develop effective care plans tailored to residents’ individual needs while ensuring HIPAA compliance.
• Supervising experience in a healthcare setting with a focus on nurse management or nursing administration roles.

Responsibilities

• Lead the design and execution of comprehensive dementia care programs tailored to residents’ needs.
• Supervise and mentor nursing staff, care coordinators, and support teams.
• Oversee individualized care plans incorporating medical history, physiology, anatomy, and preferences.
• Ensure compliance with long-term care regulations, HIPAA, Medicare guidelines, and related standards.
• Manage documentation reviews including medical records, care documentation, MDS, and clinical notes in EMR/EHR.
• Facilitate staff training on resident rights, medical terminology, documentation procedures, and dementia care techniques.
• Collaborate with interdisciplinary teams to optimize patient outcomes through effective shift management and resource allocation.
