IMPaCT Care in Atlanta, GA seeks an energetic leader to manage a team of Community Health Workers serving hundreds of clients annually. You will drive collaboration with local clinics and community organizations, oversight of performance data, and integration of CHWs with care teams to improve health outcomes.

The role requires non-profit leadership experience, extensive CHW/public health background, and willingness to travel across multiple locations.

Qualifications

• Leadership experience with non-profit or community-based orgs.
• Extensive CHW, public health, social work, or case management background.
• Deep understanding of healthcare and social service systems, incl. public benefits enrollment.
• Knowledge of community resources in the area and ability to travel.
• Strong data orientation and ability to use reports for performance.

Responsabilités

• Lead and support a team of CHWs, coaching to deliver client care.
• Partner with local community organizations to recruit CHWs.
• Analyze performance data and reports to monitor progress.
• Integrate CHWs with care teams in clinics and hospitals.
• Collaborate with organizations to connect clients with resources.
• Use data to achieve program goals and improve systems.
