Job summary

PAM Health is seeking an experienced Director of Case Management (RN) for a full-time, day-shift role. He/she will coordinate the hospital-wide case management program, review patient records for appropriate service utilization, and supervise case managers to ensure optimal clinical and financial outcomes.

Requirements include RN licensure in the resident state, 3–5 years in case management, and leadership experience.

Qualifications

• RN licensure in the state where the hospital resides required.
• Social workers may also be accepted in lieu of RN licensure, if licensed or hold a bachelor’s in social work, as required by state regulations.
• MSW, LSW, PT, OT, SLP and RT licensure may be accepted in accordance with state regulations.

Responsibilities

• Coordinate hospital-wide case management program.
• Oversee review of patient records to ensure proper utilization of services.
• Assist case managers with timely care coordination and discharge planning.
• Screen admissions to determine appropriate level of care.
• Supervise case managers and daily operations of the department.
• Contribute to fiscal planning and regulatory compliance.
• Analyze systems and variances to identify improvement opportunities.
